Medical errrs (ME) are amng the mst imprtant patient safety challenges facing hspitals and healthcare systems nwadays. Since the Institute f Medicine (IOM) reprt in 1999 “T Err is Human,” an increasing number f studies have shwn hw cmmn and deleterius ME are, especially in hspital medicine. With this, healthcare leaders invested time and resurces tward identifying and reducing ME.
A medical errr is defined as “an incidence when there is an missin r a mistake in planning r executin that leads r culd lead t unintended result.” While the majrity f ME d nt lead t an apparent adverse effect, a significant number f patients either suffer a permanent injury r death frm ME every year in the United States and arund the wrld as a result f thse errrs.
Medical errrs are the third leading cause f death in the United States after heart disease and cancer. It is estimated that mre than 200,000 patients die annually in the United States frm ME. Furthermre, in additin t the harm inflicted n patients, medical errrs are assciated with an increased healthcare cst. In a 2008 reprt, it was estimated that medical errrs cst the healthcare system in the United States mre than 17 billin dllars annually.
The first step in cmbating ME and imprving patient safety is t study the different types f medical errrs t better understand why medical errrs happen. The causes, types, and rates f ME can vary frm ne institutin t the ther and change ver time, especially as we implement changes in ur healthcare delivery. Therefre, it is imprtant t capture, track and analyze all medical errrs as pssible at the institutinal level.
As mst f the nnmedicatin medical errrs are hard t capture electrnically and manual chart review is bth unmanageable and time cnsuming, self-reprting is still the mst reliable apprach t capturing ME. Unfrtunately, underreprting f ME is a cmmnly reprted challenge even when healthcare institutins mandated reprting. While there is n agreement n what defines “underreprting f ME,” it cmmnly refers t the lack f reprts n significant ME events.
What des the underlined wrd “deleterius” prbably mean in the first paragraph?
Imprtant.B. UsefulC. Harmful.D. Adequate.
25. What is the wrst effect f ME?
A. Annual patients deaths.B. Heart disease and cancer.
C. Increasing healthcare cst. D. Destructin f healthcare system.
26. What is the first step t fight against ME t imprve the patient security?
A. T d research n the different types f ME.
B. T figure ut the reasn why ME happens.
C. T identify different institutins ver time.
D. T analyze the changes in the health delivery.
27. Why is self-reprting still the mst reliable way t capture ME?
A. Because less typical medicatin ME events recrded.
B. Because the patients are willing t ffer the reprt f ME.
C. Because healthcare institutin gives the authrity t the patients.
D. Because there is difficulty in electrnic capture as well as manual inefficiency.

Fr many f us, Tide is the mst squeaky-clean f hme helpers. This year, the 4,000 Americans selected this super-cleaner nt nly as the mst trusted detergent brand but als as the single mst trusted brand in the Hme and Family prducts categry.
When Cincinnati-based manufacturer Prtect & Gamble (P&G) set ut t create Tide in the 1930s, it was referred t as Prject X. The cmpany first sld sap and candles, made frm the animal fats. But as the 20th century tk shape, kersene replaced candles, and plant-based cleansers such as Ivry Sap replaced thse made frm animal fats. Prtect & Gamble was seeing a huge chunk f business being washed dwn the drain. The hpe was that the newfangled, entirely synthetic Prject X wuld save it.
“This may ruin the sap business,” P&G’s chairman William Gamble said at ne pint. “But if anybdy is ging t ruin the sap business it had better be Prtect & Gamble.”
At first, many f the frmulas cleaned clthes but left them like sandpaper. It tk Byerly and the Cincinnati chemists ten years t cme up with a slutin. They finally hit upn a mixture that creates mlecules that grab hard nt nly t grease and dirt but als t water. It all just washes away, leaving clean fabric withut any sapy remaining. And Tide was an instant success.
Sld first as pwder, the prduct was a huge hit. P&G’s marketing department leaped int actin, giving bxes with washing machines and flding magazines and newspapers with ads shwing happy and healthy husewives literally hugging the icnic range bx.
Then it wuld be turned int a liquid (1984) and later the fantastically prfitable Tide Pwer Pds (2012). The cmpany has develped an almst endless series f varieties: Tide with special scents, Tide with fabric sftness, and, mst recently, high-pwered Tide fr envirnmentally friendly cld-water washes.
And sn, perhaps, the newest variety might be Space Tide, the prduct f a new partnership between P&G and NASA. Tide is taking a test run t the Internatinal Space Statin t see hw well the detergent cleans and reacts in the lw-water, n-gravity envirnment utside Earth’s atmsphere.
What can be learned frm William Gamble’s wrds?
The cmpany P&G must be the first ne t ruin the sap business.
The cmpany P&G sclds smene else fr ruining the sap business.
The cmpany P&G shuld be t blame fr the ruin f the sap business.
The cmpany P&G has cnfidence and pwer t refrm the sap business.
29. Hw many series f Tide prduct variety have been prduced since 2000?

The wrld is reliant n the climate mdels that frecast ur warming future and guide climate actin. But why are climate mdels s cmplicated? Paul Ceppi, a lecturer in climate science at the Grantham Institute, explains mre as fllws.
There’s several layers f uncertainty in making climate predictins. The first ne is just knwing hw much greenhuse gases will be emitted. That’s why the IPCC cnsiders a range f future emissin scenaris. But then within each scenari, there’s an uncertainty that cmes frm physical prcesses taking place in the atmsphere and the ceans.
I wrk n climate sensitivity, which is this basic questin: hw much glbal warming can we expect fr a given increase in CO2? And t answer that questin, we find that a lt f the uncertainty cmes frm cluds. Even fairly subtle changes in the amunt, r type, r ther prperties f the cluds can have a pretty large effect n the amunt f warming.
We can expect cluds t change due t glbal warming, and in turn thse changes culd accelerate warming further. There are tw main changes. One is that with glbal warming, we expect the amunt f lw cluds t decrease. Because cluds act a bit like an umbrella and reflect sunlight back t space. If we have fewer f thse umbrellas, then the surface ends up warming mre as it absrbs mre sunlight. And then the ther is that high cluds will rise t even higher altitudes. That will increase their greenhuse effect, because as well as reflecting sunlight, cluds als have a greenhuse effect f their wn. It culd be described as blanket. If cluds rise, this blanket effect actually increases.
These effects were already in the mdels, just with large uncertainty. S n average, it seems that mdels were actually getting it right. We’re nt suddenly finding that they are ging t be a lt wrse, but it’s mre that we’re able nw t reduce the range f uncertainty in these predictins.
Paul Ceppi explains that fr the climate scientists, it’s a challenge smetimes t talk abut these uncertainties. We want t be dwnright, but at the same time we dn’t want t appear as if we dn’t knw what we’re talking abut. That’s why I think it’s als imprtant t talk abut the knwn facts, fr example, that increasing greenhuse gases des cause glbal warming: there is abslute cnsensus(共识)n that.
Which f the fllwing statement will the authr agree with?
Cluds mainly lead t the uncertainty f glbal warming.
The higher altitude the cluds rise, the less greenhuse effect is.
The scientists have knwn the amunt f the greenhuse gases emissin.
The scientists are unable t reduce the range f certainty in the climate predictin.
What is the authr’s purpse f writing paragraph 4?
T shw the difficulty in figuring ut the uncertainty.
T call n peple t be aware f envirnment prtectin.
T teach peple t learn mre abut the findings n cluds.
T intrduce the interactin effect between cluds and climate.
What des the underlined wrd “they” in paragraph 5 refer t?
A. Mdels B. Predictins.
C. EffectsD. Uncertainty
What’s Paul Ceppti’s attitude t the uncertainties in making climate mdels?
A. Hnest.B. Puzzled.
C. Indifferent. D. Dubtful.
