Reading
When a patient is admitted t the hspital, he is usually taken t a ward, where he is asked t give his name, age, prfessin, and address. At the same time admissin nte will be taken.
Firstly, the patient will take rutine examinatin: temperature, pulse and respiratin are taken and recrded.
Then the patient is taken t the bathrm where he is given a general bath and is helped t put n a hspital gwn. Sme patients wh can’t mve may be given a spnge. A spnge, which is a bed bath, is usually given t patients cnfined t bed.
After the abve prcedure the patient will be taken t the different wards accrding t his disease, where he is questined by a ward dctr abut his present cmplaint, the date f the nset f the disease, the previus illness and the family histry.
Then the ward dctr prceeds t d the physical examinatin f the patient. If it is cnsidered necessary, X-ray examinatin is made t. Bld pressure is measured and bld, urine and ther kinds f labratry analyses are made fr all new patients n admissin.
Writing an admissin nte is the mst imprtant daily wrk f a ward dctr. The fllwing is an example f admissin nte:
A 30-year-ld married female, X X X, was admitted t the hspital n August 3, 1996, because f a 7-day fever and cugh.
Chief cmplaint: Run a fever and have a cugh fr seven days
Present illness: Seven days ag she began t have a fever, cugh and spit mucus sputum, but she had nt received any medical treatment up t nw. In the past three days, she had a persistent fever, and the cugh became serius. She spit rusty sputum. She felt chest pain and it intensified with respiratins. During her illness, she was lack f strength. Her appetite was pr. Her urine and stls were nrmal.
Past histry: She was healthy in the past and seldm went t see a dctr.
Family histry: Nbdy in her family had ever gt tuberculsis and she did nt knw f any clse cntact with patients with this disease either.
Physical Examinatin: The patient was nrmally develped. She lked acutely ill but her mind was still clear. There was n eruptin, r purpura ver skin. Superficial lymph ndes were nt enlarged. Skull and head rgans were nrmal and there was n jaundice n sclera.The neck culd mve freely and thyrid glands were nt engrged. Chest was symmetrical. Tactile fremitus exaggerated slightly ver the right upper chest and a lt f small and mderate mist rales were heard. Heart beats were nrmal withut any nise. Abdmen was flat and sft and free frm tenderness. Liver and spleen culd nt be palpated. Bwel sunds were nrmal. There was n abnrmality in spine r limbs. Knee jerks f bth sides were active. There n pathlgical reflexes.
Rutine bld test: RBC: 4500000/mm3, Hb: 140.5g/L, WBC: 12000 /mm3, N: 80%, L: 19%, E: 1%
Platelet cunt: 150000 /mm3
Chest flurscpy: There was a big patchy shadw ver upper right lung.
The admissin nte shuld be highly prfessinal, first and fremst, highly specialized terminlgy. These terminlgies are accurate, standardized, and unified s as nt t cause any ambiguity in cmmunicatin.
Ntes
A spnge, which is a bed bath, is usually given t patients cnfined t bed.
