These vulnerable individuals struggle with securing apprpriate fd, clthing, and shelter due t their mental illness. In additin, they may have critical shrtage in cmmunicatin, and are generally highly avidant f medical services. They are unable t live safely in the cmmunity and require specialized mental health services t secure and sustain husing.
Mst referrals (移交) are submitted by hmeless utreach prgram staff members wh identify individuals with severe impairments that require specialized and intensive supprt and engagement.
The general public may initiate a referral t a hmeless utreach team thrugh the Ls Angeles Hmeless Outreach Prtal at www. LA-HOP. rg. Individuals will be assigned t a HOME team if the persn meets the criteria.
