Step I The purpse f the survey（Read and find ut）
Yur schl wants t start sme new sprts clubs and wants t knw which clubs wuld best fit students’ interests and needs.
【设计意图】做调查首先要弄清目的。
Step II Cllect questins（Read and add）（SB，P45, Part 1）
Ask students t wrk in grups f five. First read the questins and then add ne mre that yu wuld like t ask yur classmates accrding t their interests and needs.

2.At what time f day d yu like t d sprts?
3.Hw ften d yu ________ every week？

【设计意图】一次高效的问卷调查的关键是看问题的设置，不同小组设置的问题也可能不同。因此建议在调查之前共享问题，尽量分成不同类别，提高调查的可操作性。
Step III Cnduct the survey(SB, P45, Part 2)
1. Chse the subject f survey.
Each grup member chses three classmates as the subject f survey.
2. Ask and write dwn.
Ask the grup member t ask the questins frm the list abve. Write dwn their answers in the table.
【设计意图】采用表格的形式使数据思路更清晰，但在记录时要注意被采访者的言外之意，对问卷分析能起到很好的作用。
Step IV Write up the survey results(SB, P45, Part 3)
Ask students t use the data t write up the survey results. Be sure t include the fllwing pints.

Step V Cmplete the reprt(SB, P45, Part 4)
Ask students t wrk in grups and cmplete their reprt.
EXAMPLE
This week, ur grup f five students did a survey ur classmates t find ut what sprts interests and needs ur students have. We hpe that the results f ur survey will be useful t the schl in deciding what new sprts clubs shuld be set up.
The first questin we asked was, “What is yur favurite sprt? ”Eight ut f 15 students said that playing ne-n-ne sprts is their favurite way t get exercise, like ping-png r badmintn. Five ut f 15 said that they prefer team sprts, such as ftball. Only tw said that they like t exercise alne, such as jgging r walking.
